Misunderstandings in the application of laryngeal mask airways

Can laryngeal mask airways be used for laparoscopic surgery? We need to answer two questions: First, can the laryngeal mask airway remain ventilated when the pressure in the abdominal cavity rises? Second, is the laryngeal mask airway safe when the pressure in the abdominal cavity rises?

The routine operation of laparoscopic surgery is to fill the abdominal cavity with carbon dioxide, which will cause increased intra-abdominal pressure and upward movement of the diaphragm, reduced chest and lung compliance, increased airway pressure, etc., thereby affecting the patient’s ventilation function, so laparoscopic surgery Respiratory management is more demanding. The traditional view is that endotracheal intubation is the safest method for laparoscopic surgery. The first-generation laryngeal mask has poor airtightness and is not widely used in laparoscopic surgery. Some even believe that laparoscopic surgery is due to intra-abdominal pressure. Too high makes the use of laryngeal mask taboo, but these views lack sufficient evidence to support them.
